for Physicians add $ 150
Evening Lectures (Wednesday, Thursday
& Friday) will be open to non-conference participants on a space available basis
for a fee of $30.00 per lecture. Registration for evening lectures will be
accepted, if available, beginning September 2.
Single-day Registration will only be
available if the Conference does not sell out. The one-day registration fee is
the same as the Saturday Nov. 8/Medicine Buddha Teaching. Registration for
single days will be accepted, if available, beginning September 2.

PLEASE NOTE: All Breakout
Sessions must be registered for in advance. Early registrants will receive a
copy of the conference schedule by e-mail or first class mail as soon as it is
finalized and be given the opportunity to register first for the sessions of
their choice.
CONFIRMATION:
When your registration has been
processed you will receive a confirmation letter by mail or email. On arrival at
the hotel, please sign in at the conference registration desk where you will
receive your badge, program, information and other materials.
CANCELLATION:
All cancellation requests must be made
in writing. No phone cancellations will be honored. A full refund minus a $50
processing fee will be made for each request postmarked by October 27, 2003. No
refunds will be made for requests postmarked after October 27, 2003. All refunds
will be issued after the conclusion of the conference. Please send your written

EDUCATION CREDITS FOR PHYSICIANS:
Beth Israel Medical
Center and St. Luke’s-Roosevelt Hospital Center are accredited by the
Accreditation Council on Continuing Medical Education to sponsor continuing
medical education for physicians and take responsibility for the content,
quality and scientific integrity of this CME activity.Certification: Beth

18 credit hours in Category 1 towards the Physicians Recognition Award of
the American Medical Association. Each physician should claim only those hours
